Investigation Summary

Investigation Nr: 202456893
Event: 09/14/2010
Employee Suffers Fractures in Fall from Roof

At approximately 2:00 p.m. on September 14, 2010, Employee #1, a carpenter with Allen Construction, Incorporated, was working from the floor of a second story single family residence being constructed on site. Another coworker was installing the roof sheathing of the second story and Employee #1 was handed a starter board to install. Employee #1 stepped out of a window opening onto the first story roof in order to inspect the roof joists and corbels for proper alignment. The roof sheathing on which Employee #1 stood was installed at a slope of 20 degrees (approximately 5:12). Employee #1 stated that he lost his footing, due to slipping on wood dust or shavings, which caused him to slide off the roof. He fell 9 1/2 ft to the soil grade. Employee #1 was transported to the hospital and treated for multiple unspecified fractures. The project foreman was Employee #1's Direct Supervisor and was on site, but not in the vicinity of the incident. Cal/OSHA determined that the incident was caused when Employee #1 lost his footing, due to the presence of wood dust or chips on the roof sheathing. No violations of Title 8 regulations were identified regarding the incident and no accident-related citations will be issued to the employer.
