Investigation Summary
At approximately 2:30 p.m. on July 22, Employee #1 was performing his duties of employment as a demolition helper. Employee #1 was transporting plywood roof sheathing from the roof of a building on the west side of the site. The plywood was being removed from the roof of a building using a high reach industrial truck, and Employee #1 was carrying plywood to the location where it was to be lifted off. According to the project manager, Employee #1 was walking with a piece of recovered plywood when the beam he was walking on, became detached from the support joists, causing him to fall to the ground.
