Investigation Summary
On December 17, 2009, Employee #1 was responding to a service call at the Torn and Glasser company, which is engaged in food processing, including dry roasting various types of nuts. There was a reported squeaking noise coming from the top of the Proctor roasting machine. Both he and a coworker got up on top of the roasting machine to determine the cause of the noise. They attempted to trouble shoot the problem, when he inadvertently placed his left pinky finger in the belt and pulley drive of the recirculation fan motor. From the investigation, it was determined that he had asked another coworker on the production floor to turn the motor off after diagnosing the source of the noise. While the motor was coming to idle, his left pinky got caught by the belt and pulley drive, amputated it. He was taken to The Hand and Wrist Center, where he was treated and released.
