Investigation Summary
At approximately 3:00 p.m. on July 31, 2009, Employee #1 was cleaning head Number 4 of rice cake machine Number 7. The heads of each rice cake machine pop out potato chips and are guarded by a chute that also allows for the chips to slide down to a small conveyor. In order for Employee #1 to clean the potato chip debris, he had to use an extension tool and a small burst of compressed air to clear the machine for proper operation. Employee #1 turned off the machine, removed the chute from head Number 4, and cleared the potato chip debris. He reached up to turn the machine on with his right hand, prior to replacing the chute, unaware that his left hand was slightly inside the point of operation. Employee #1 had been trained to put the chute back onto the head of the machine prior to turning it on. When he turned the machine back on, the tip of his left ring finger became caught in the machine. He suffered a crushing injury, and was taken to the hospital, where his fingertip was amputated. He was hospitalized for treatment.
