Investigation Summary
Employee #1 was walking from the coal handling facility to the scrubber area, to help with an electrical problem. As he passed by the outside area of the coal belt junction tower, a large piece of ice fell from the bottom structure of the tower, and struck the employee. The ice was either very dirty or was covered with coal dust and was approximately 50 inches long by 25 inches wide. The employee was struck on the back of his head and the neck and was killed.
