On November 3, 2010, Employee #1 was cleaning up an area were two reinforced steel bridge plates, approximately 8 ft by 11 ft and weighing 1,000 pounds, were leaning against an interior wall. One of the bridge plates fell and struck Employee #1 who was hospitalized and died several days later. .
