Emplolyee Seriously Injures Hand in Shaft

At approximately 4:15 p.m. on June 7, 2010, Employee #1 prepared to polish a diameter shaft with an emery cloth using a manual, horizontal lathe in a machine shop setting. Employee #1 removed a glove from his right hand as the left glove remained on his hand. Employee #1 then secured the shaft in the lathe using a collet, and obtained a strip of emery cloth. Employee #1 started the lathe according to procedures. As the part rotated in the lathe, he applied the Emery cloth to the work piece by holding one end of the strip in each hand and applying light pressure on the work piece with the emery cloth. Employee #1 heard a noise and looked up, and as he did so, something caught and pulled his gloved left hand into the spinning shaft. As Employee #1 heard his wrist bones breaking, he applied the emergency shut off foot control and let out a yell. At the scene, Employee #1's left hand received an amputation to part of the lower wrist and the index, middle, and ring fingers. At the hospital, the fourth digit was removed in surgery.
