Investigation Summary

On June 8, 2009, a crew of three employees was lifting an advertising sign. The sign measured approximately 10 ft. long x 6 ft. wide and was attached to 2 vertical legs and was made of heavy steel. As the workers began to raise the one end of the sign, the weight became too great, and the sign began to slip and fall away from the employees. All three of the employees were standing on the same side of the sign, as the sign started to fall Employee #1 who was standing in the middle fell directly underneath a leg of the sign and was struck on the head and died.
