Employee Falls from Roof

On February 11,2011, Employee #1 was seriously injured when an improperly angled and unsecured portable ladder slipped down and back, as he stepped off of it onto a garage roof. Employee #1 fell backwards over eight feet onto the concrete driveway below, landing on his SCBA tank and cracking two vertebrae. Employee #1 was taken to a hospital where he was admitted and treated for three days.
