Investigation Summary

Investigation Nr: 202490264
Event: 03/02/2010
Employee Amputates Thumb Tip in Case Sealer Malfunction

On March 2, Employee #1 was assigned to remove cases of finished goods as they exited a Supertaper ST1A case sealer. Small bags of prunes were placed into cardboard boxes and the boxes go through the case sealer which folds the flaps down and secures them via a strip of tape. On the day of the accident, Employee #1 reported that the tape dispensing component was malfunctioning. Another malfunction occurred and Employee #1 stuck his left hand into the case sealer while it was still operational (though no boxes were moving through it). Just as Employee #1 grabs the end of tape, boxes enter the case sealer and a knife used to cut the tape comes down, striking Employee #1's thumb and amputating the tip. After removing his hand from the unit, Employee #1 was taken by a coworker to a nearby clinic then transferred to Rideout Memorial Hospital in Marysville where he was released several hours later. Employee #1 was a utility worker at the Sunsweet Growers, Inc. facility in Yuba City.
