Employee Is Injured in Fall from Truck Bed

On July 29, 2009, Employee #1 was working alone delivering tires to stores. Apparently, he either fell or stepped off the back of the truck and fell about 3 ft to the parking lot. Employee #1 was hospitalized for a concussion. He does not recall how he fell. There were no witnesses.
