Event: 07/15/2009
Laborer Breaks Writs and Elbows Falling Off Stack of Pallets

At approximately 6:20 a.m. on July 15, 2009, Employee #1 who worked for a staffing service as a general laborer was unloading a truck of coconuts. Workers were unloading 100-pound bags of coconut from a trailer container, Employee #1 was standing on a stack of pallets outside the container when the load of bags that they were positioning for the forklift, shifted, causing the bags to slide forward and knock Employee #1 off the pallets. Employee #1 fell approximately 4.5-feet to the concrete below and injured both arms. According to the Forklift Operator, it was common practice for workers to stand on a stack of pallets while unloading the container. The supervisor for the temporary workers on-site that day stated he did a basic safety review of the job with the workers prior to starting. It was reported that Employee #1 broke elbows and wrists in the fall.
