Investigation Summary
At approximately 3:05 p.m. on August 23, 2010, Employee #1 had delivered milk to a cheese processing plant via a Sterling bulk milk hauling truck. After delivering the milk, he conducted internal and external tank washing operations from a top of the truck and within Bay Number 2. He was walking on top of the smooth rounded stainless steel bulk tank while not wearing a personal fall arrest system. The indoor truck bays were equipped with retractable lanyard style personal fall arrest systems. Upon completing the washing activity, he hung up the hose above the truck and turned when he lost his footing. He fell approximately 10 ft and struck his head and left shoulder upon a concrete surface. Employee #1 lost consciousness. Emergency medical personnel were summoned. They arrived and performed first-aid and other treatments. He was airlifted to hospital where he died on August 26, 2010 because of massive traumatic head injury.
