Investigation Summary
On May 13, 2011, Employee #1, a groundskeeper, was in trimming a tree that measured an approximate height of 30 feet. While climbing and maneuvering amongst the branches, one of them broke. Employee #1 fell about 15 feet, striking an asphalt surface. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for serious head trauma. During the accident investigation, it could not be determined if he was standing on the branch that broke or if his safety line was anchored around that branch. Also, with no eye witness, it was inconclusive as to whether he was using his protective equipment properly.
