Employee Falls from Ladder and Fractures Foot

On March 30, 2011, Employee #1 was engaged in the refurbishing of a cooling tower. He was occupying an unsecured extension ladder at an approximate 6 ft height. As he completed his maintenance work by wiping down the top edge of a rail, he extended his reached to the right, which caused the ladder to tip over. He fell approximately 6 ft onto a floor, suffering a fractured right foot. Employee #1 was transported to a medical center, where he underwent surgical procedures for the fractured foot and then received post-operative care and was hospitalized.
