Investigation Summary
On March 18, 2011, Employee #1, a full-time, non-contracted line supervisor, was working for his employer, a manufacturer of frozen cookies and pastry. Employee #1 noted that excess flour was not being removed from the dough running down the line, due to a blockage in the Moline flour removal auger. He turned off the machine and then reached under the guard to adjust the cover over the auger and remove the impediment. The auger blade, which was still spinning, cut off the tip of his right ring finger. The employer and emergency responders reported the injury to the Division on March 18, 2011.
