Investigation Summary
On January 19, 2011, the fire department called the Division to report a serious incident that occurred the same day. Employee #1 placed a FNB -83 nickel hydride battery, rated at 7.2v, 2000 MAH into a battery analyzer. The analyzer, model VIa was manufactured by W&W Manufacturing, maximum capacity was 9999 MAH. It was in charge mode. The battery caught fire then popped, a minor explosion took place. Employee #1 took a step towards the analyzer to turn it off when another pop sounded. Employee #2 pulled a fire extinguisher and put out the fire. When the second explosion took place, Employee #1 fell back fracturing his femur. Employee #1 indicated that he had been performing similar work for 18 years and this hadn't happened to him before. Employee #1 was hospitalized 7 days undergoing surgery to repair a fractured femur at a nearby Hospital.
