Investigation Summary

Investigation Nr: 202552410
Event: 12/01/2010
Employee Suffers Seizure and Dies

On December 1, 2010, Employee #1 was sitting at a table in the dining area of the warehouse having lunch, and he suffered a seizure. Employee #1 did not fall to the ground during the tonic clonic phases of the seizure, but instead remained seated with his upper body and head stumped over on top of the table. It was during the postictal state of the seizure that other employees noticed that he was unconscious and not breathing. The counseling staff and management personnel moved him to the ground and administered cardiopulmonary resuscitation. They summoned emergency medical personnel. He was treated at a fire department and was then transported to a hospital. Employee #1 was pronounced dead at the hospital at 12:55 p.m. by the certifying medical doctor. The cause of death was due to cardiopulmonary arrest and epilepsy.
