Investigation Summary
On September 24, 2010 Employee #1, a carpenter, had been forming walls and pouring concrete. Employee #1's supervisor stopped to check the work and observed Employee #1 shaking, and not answering questions coherently. The supervisor immediately put Employee #1 into the shade, provided cool bottled water, and called 911. Employee #1 was hospitalized four days with a diagnois of heat stroke.
