Investigation Summary
On July 15, 2010, an employee was working in the metal recycling yard where he was assigned the tasks of sorting out different types of scrap metal pieces. There were some large metal pieces which required to be cut into smaller pieces. In order to cut them, the employee had to use an acetylene torch. After turning on the acetylene and oxygen supplies, he ignited the torch. As he was adjusting the torch flame, the acetylene hose ruptured at the coupler connecting the hose into the torch, it caught fire and began whipping around. The employee could not get away and he got on fire. The employee suffered third degree burns to his left arm and to his left side of the body under the arm. He was treated and hospitalized for 12 days.
