Investigation Summary
On June 10, 2010, Employee #1 was cleaning the juice bar area and attempted to remove a 12 lb. glass barrel container from the ice. During this process, the glass barrel container slipped out of his hands, as they were attempting to grab the container, it cracked on the ground and lacerated the tendon of the right hand of Employee #1. Employee #1 was then transported and admitted for in-patient surgery at Western Medical Center.
