Investigation Summary
On June 2, 2010, Employee #1 was attempting to remove a three phase, three pole breaker from an energized buss bar assembly. Employee #1 was holding the breaker with his left hand while at the same time attempting to loosen the top screw on the right side of the breaker with his right hand, using a flat head screwdriver. When the top screw was completely removed from the threads, the breaker rotated to the left, allowing the three metal mounting poles to short between the three poles of the breaker mounted on the opposite side of the switchboard. This electrical short caused an intense arc flash, resulting in a severe burn injury to the left hand of Employee #1.
