Investigation Summary
At approximately 9:45 a.m. on March 16, 2010, Employee #1 was moving some boxes, office materials and file folders. In the process of carrying file folders, Employee #1 lost her footing, tripped and fell forward onto a desk and then to the ground. To break her fall, she extended her arm out and subsequently, dislocated her shoulder and fractured her arm. Employee #1 was then transported and admitted to Anaheim Medical Center, where she was hospitalized and underwent surgery.
