Employee's Finger Is Amputated in Mixer

On November 16, 2009, Employee #1, a warehouse worker, was assisting the operator of a horizontal mixer (Model Number 42A-18, Serial Number 0604277). This machine was 60 in. long by 24 in. wide by 27 in. deep. The bottom of the mixer, which was approximately 3.5 ft above the ground, had a 4-in.-diameter hole where the product exited. Employee #1 was observing as the product exited to ensure that it entered a receiving container. When he noticed that the product flow had stopped, he notified the operator. Employee #1 then ascended a ladder to investigate. He noticed that a clump of product was blocking the hole. He then returned to the bottom of the mixer and reached into it with his hand to free the clumped powder. Employee #1's finger tips were amputated. He was hospitalized for treatment.
