Investigation Summary
At 2:53 p.m. on November 17, 2009, Employee #1, a SWAT team member from The SWAT Division of the City of Santa Ana Police Department, was involved in a "possible armed gunman with hostages" situation. Other employees were positioning themselves for possible entry into a building, when Employee #2 accidentally discharged his weapon. As a result, Employee #1 was struck in the right foot by a bullet. The assault rifle in use was a Colt M4A1.223 caliber weapon. Employee #1 was then transported to the hospital, where he was admitted for surgery on his right foot.
