Employee Amputates Fingers While Using Table Saw

On June 30, 2009, Employee #1 and two coworkers were installing wood flooring at a single family residence. Employee #1 was using a Hitachi table saw; model C-10FR, serial number N63797 with a 10 in. circular blade. He was making a cross-cut on a wooden plank, when his left hand came in contact with the rotating blade, which was not guarded by a hood guard. Two employees did not witness the accident, but they came to his aid. Employee #1 suffered amputation of multiple fingers. His left middle finger was cut down to half its length. Left index finger was cut at its base and it was reattached in the hospital. Left thumb was also lacerated. He was hospitalized for four days.
