Investigation Summary
Employee #1 was standing on a standard rolling scaffold, installing interior insulation to a ceiling of an industrial office space being remodeled. The scaffold platform was 8-ft 2-in. above the floor level, and the supervisor was handing him the insulation and moving the scaffold, as needed. The supervisor received a cell phone call, and maintained he had told the injured employee to stay still while he answered the phone. While talking, the supervisor stated he heard the scaffold start to move, and as he turned, one of the scaffold wheels dropped into a hole in the floor that was waiting for a city inspector to approve. Although the supervisor tried to stop the scaffold from falling, he could not hold it up, and the Employee #1 fell approximately 8 ft to the floor. The scaffold platform landed on Employee #1's left leg and fractured it. Employee #1 was hospitalized.
