Investigation Summary
On July 27, 2009, Employee #1 was on a crew trimming trees at a golf course. Employee #1 was ascending a tree. Before Employee #1 had crotched a safety line, a branch that he stepped on broke and he fell 12 ft to the ground. Employee #1 was taken to a local hospital where he was hospitalized for two days with a fractured right arm.
