Employee Dies of Heart Attack at Work

On June 8, 2009, an electrical contractor was getting prepared to expand a distribution panel that would add a new 1600 amp circuit, which would be used to power two new boilers that were going to be installed during shutdown in July 2009. An employee was kneeling in front of a live motor control circuit ready to pass a rope through the bottom of the unit to pull electrical cables which were not live, to the unit. During this work, the employee fell forward into the motor control circuit and hit his head on a live circuit. The employee was killed. The opening in the panel was 16-in. by 16-in. The live circuit was approximately 9 in. from the front of the cabinet. The coroner pronounced that the employee died of a heart attack and then fell into an electrical conductor.
