Investigation Summary
At approximately 9:30 a.m. on March 16, 2011, Employee #1 was working with a team attempting to set a log barrel sorter. They were using a hydraulic crane to attempt this lift. When the lower end of the barrel made contact with the brace it failed due to lateral forces. The brace was designed for vertical forces and not lateral forces. Employee #1 was standing on the platform where the barrel was being set. Employee #2 was working from a lift while the barrel was being set. When the collapse occurred Employee #1 rode the platform down to the ground. The barrel slid through the double sling and took out the platform as it fell to the ground. Employee #1 suffered a compressed fracture in the L3/L4 vertebrae. Employee #2's lift was struck by the barrel when the collapse occurred. He sustained back strain due to the lift shaking. This accident occurred due to the inexperience of the structural design work on this site. Another contributing factor was the lack of experience and training with rigging and operating the crane.
