Employee falls during fall protection training

At 4:45 pm, on May 2, 2010, employees were going through fall protection training for communication towers. Employees were performing a mock rescue operation on a tower in a large building of the company. Employee #1 was watching the mock rescue from above at about 17 feet. All employees were wearing harnesses with vertical wire rope grabs on a wire rope. Employee #1 started his descent. During his descent, the employee fell backwards and the vertical life line gave way. The vertical life line was looped around a piece of metal and tied with a one U-bolt. The wire rope slipped through the U-bolt causing the anchorage point to fail. Employee #1 fell approximately 15 feet to the ground below, suffering a compress fracture of his T-12 vertebra. Employee #1 was hospitalized due to his fracture and injuries.
