Investigation Summary
On January 14, 2010, Employee #1 and a coworker were tightening bolts on a Norvell Company flour sifter, used to hold the machine together. While Employee #1 was standing on top of the sifter, his coworker was inside the sifter. The sifter, with its top approximately 8 feet above the floor, is supported from the ceiling by several wooden dowel rods on each corner of the unit. As Employee #1 walked from the rear of the sifter to the front, where his ladder was positioned, he fell. He was hospitalized, where he was treated for multiple fractures, as well as a brain injury. The accident investigation revealed that Employee #1 had been holding onto one of the dowel rods that helped hold the sifter to the ceiling, when it broke and caused him to fall.
