Employee Is Killed When Pinned Between Truck and Dock Bumper

On June 23, 2009, Employee #1, an independent contractor, was escorting a container for inspection. Prior to leaving the warehouse, the driver of the truck pulled it back to allow the warehouse to place the seals on the container. Employee #1 was positioned behind the container and was pinned between the back of the truck and the dock bumpers. He died from his injuries.
