Investigation Summary
At approximately 6:45 a.m. on May 29, 2009, an employee was riding the back of a 1993 Ford Truck, Florida License X12-4AA. He apparently lost his balance and fell onto the pavement. The employee was hospitalized but died on June 12. The employee was not wearing any type of fall protection at the time of the accident. Company records indicated that the employee had been trained on the company's policy which prohibited riders on all type of moving equipment. The employee had received and signed acknowledgement form with regard to the company's prohibition policy about riding moving equipment. The employee drove a water truck and was on site as part of his routine activities. From witness interviews, it was learned that the employee was walking the site and jumped on the back of the truck as it passed him by. There were four other company coworkers on site, none of which worked directly with the employee. According to one witness, the employee fell onto the pavement but did no try to protect himself against injuries during impact.
