Investigation Summary
On March 14, 2009, Employee #1 was a parking lot attendant, directing traffic flow at a parking garage entrance. He was struck by a scooter which knocked him to the ground. He struck his head, causing a head injury. He was hospitalized and placed on life support. He was removed from the life support machine and died on March 25, 2009.
