Employee Is Pinned Between Two Booms and Is Killed

On August 26, 2009, Employee #1 was supervising scrap metal salvage operations. Two track hoes with electro-magnet attachments were being operated by coworkers #1 and #2, in close proximity to each other. A chain tangled on one of the magnets and coworker #1 grounded his magnet and boom for the employee to pry the chain loose with a crow bar. The employee realized the other track hoe was too close and ordered coworker #2 to move the machine. The employee turned back around to continue working on the machine. As coworker #2 swiveled the machine to drive forward, the counter weight pinned the employee against the grounded boom of the first machine. Employee #1 was killed.
