Investigation Summary
At approximately 8:30 a.m. on February 21, 2012, Employee # 1 was working for W. W. Felix Builders. Employee # 1 and crew were working inside the trench - two were using electric breaker hammers to undercut a trench wall (non-property-line side), and the other two were using shovels to remove the excavated soil. Employee # 1 was hit by falling debris while working in the trench.
