Investigation Summary
On August 29, 2011, Employee #1, a delivery driver, was driving a cab-over truck (14,500-lb GVW) equipped with a refrigerated box to deliver macadamia nut products to stores. He was alone in the vehicle, driving back to the processing plant after making his deliveries for the day. The two-lane road was dry, straight, and generally level with open and level shoulders. For unknown reasons, the truck crossed the centerline, went off the road and collided head-on with a tree, severing its trunk and extensively damaging the truck. Employee #1 was pinned in the cab and died at the scene from multiple injuries. The police inspection of the truck found no mechanical defects to account for its crossing the centerline. A witness, who was stopped on a side road, stated that Employee #1's truck was crossing the centerline as it passed, and he saw him slumped to the right with his eyes closed.
