Investigation Summary
On January 22, 2011, Employee #1, a tree trimmer, was accessing a tree. He climbed and stood on the top rung of an unsecured extension ladder and was in the process of throwing his climbing rope into the crotch of the tree. The throwing action of the rope caused his body to shift its weight and lose his balance. He fell to the ground. Emergency medical personnel were summoned. Employee #1 was transported by ambulance to a medical center, where he died upon arrival.
