Employee Falls from Elevated Pallet and Later Dies

At approximately 7:56 a.m. on March 13, 2010, Employee #1 was working on a wooden pallet lifted by a company forklift truck. When the forklift truck moved in reverse, the employee fell from the pallet, 15 ft. Coworker #1, the forklift operator, saw the employee laying face down on the concrete floor bleeding. Employee #1 was transported to Queen's Medical Center, where he died approximately thirty-two hours later.
