Investigation Summary
At approximately 3:34 p.m. on November 2, 2009, Employee #1 was a mechanic. He was working at the rear of an electric forklift. The forklift unexpectedly moved in reverse. Employee #1 was crushed between the rear of the forklift and the rear of a parked flatbed truck. He was killed.
