Investigation Summary
On February 10, 2009, Employee #1 was a inspector for a company that performed special inspections. He was on the roof of a four story residential building being constructed. The roof had a 5:12 pitch. He was expecting the nail pattern for the roof sheathing and was walking along the ridge. He lost his footing, slid down the roof, and fell approximately 40 to 45 ft to the ground. Employee #1 died at the hospital. He had not been wearing a harness with a secured lanyard nor was he otherwise protected from fall hazards while performing this job.
