Investigation Summary
At about 2:50 p.m., on April 15, 2009, employees of Peak USA Energy Services were tearing down an oil drilling rig. The crew was loading the derrick onto a semi trailer. Employee #1 was on top of the trailer to chain down the derrick when a chain broke that was holding up a cylinder. The cylinder fell and crushed the employee.
