Investigation Summary
At approximately 6:27 p.m., on September 8, 2010, Employee #1, a flagger, was working with a crew filling cracks in a road. They were working on a flat straight stretch of road with approximate visibility of quarter of a mile. Employee #1 had one car stopped in front of him. An empty 2-axle grain truck was approaching the area and did not appear to notice the work zone or to be slowing down. As the truck was upon the area, Employee #1 moved toward a ditch. At the same time, the driver of the truck swerved the truck toward the ditch, striking Employee #1. Employee #1 was killed.
