Employee Is Killed in Fall from Ladder

On December 23, 2008, Employee #1 was working from a stepladder approximately 4.75 ft high to grind and weld the inside top of an under construction feeder bin. The bin was placed upside down and was sitting on steel sawhorses (approximately 2.08 in. above the concrete floor). As Employee #1 was grinding, his air hose became loose. A coworker traveled over to Employee #1 to talk with him. Employee #1 reattached the hose. He soon began descending from the ladder. The ladder kicked out, and he fell backwards, hitting his head on the concrete floor. Employee #1 was transported to a medical center, where he died.
