Worker Amputates Thumb While Using Unguarded Table Saw

On February 2, 2009, Employee #1, of Urethane Unlimited LLC., was cutting Styrofoam insulation board. To cut the board he used a Craftsman 3.0 horsepower table saw (Model Number: 137.248840, Serial Number: RJZ1254). While cutting the sheets of Styrofoam insulation board, Employee #1 came into contact with the exposed blade, amputating his right thumb and part of his pinky finger. He was not hospitalized for his injuries.
