Investigation Summary
On March 18, 2011, Employee #1, a connector, was preparing to connect a structural steel beam to installed steel columns. He was located atop a steel beam, waiting for the beam to be positioned with a crane. Employee #2, another connector, was located at another column to connect the opposite end of the beam. The crane positioned the beam, and the connectors began their work. The column closest to Employee #1 collapsed, causing Employee #1 to fall approximately 50 feet, as the beam to which he had connected his fall protection collapsed. Adjoining columns also collapsed. Employee #2 was able to remain aloft, and was rescued from the collapsing structure by the crane operator. Employee #3, a laborer performing work on the ground, observed the collapsing steel and attempted to run from falling structural steel. Employee #3 injured his leg while avoiding being struck. Employee #1 was pinned beneath a collapsed column. Employees #2 and #3 suffered leg injuries. Employee #1 suffered blunt force trauma injuries. He died from his injuries on March 19, 2011.
