Investigation Summary
On September 2, 2010, Employee #1, a truck driver, was tying down a load of orange bins onto his trailer. He was standing on the opposite side of the truck trailer, adjusting the ropes, when a forklift operator began loading a stack of bins onto the trailer from the other side. The forklift operator set the stack down, pushing the bins onto Employee #1. He sustained a fracture to his knee and was taken to a nearby medical facility, where he was hospitalized for treatment.
