Investigation Summary
On June 18, 2010, an employee, a carpenter, was installing a carpenter's bracket scaffold manufactured by JAX Scaffold Systems at a two story multiple unit new construction project. The scaffold collapsed when the blocking to which the hanging bracket was secured failed, resulting in the employee falling 16 feet. The employee was hospitalized and treated for multiples fractures The blocking was found to be attached by two diagonally driven nails into the studs. The cause for the scaffold collapse was determined to be the hanging bracket that was not secured to an adequately strong structure member in compliance with 8CCR 1645(e)(2)(D) and the manufacturers' installation guide.
