Investigation Summary
At approximately 7:30 a.m. on May 11, 2010, Employee #1 was on top of a tanker trailer, removing hoses used to purge the sections inside. The trailer was not properly balanced and fell over onto its' side. Employee #1, jumped from the trailer and incurred broken ribs, sternum and vertebrae, emergency response was called to the scene.
